Starting with Ectopic Pregnancy[edit | edit source]
We began our journey with addressing the laparoscopic treatment of ectopic pregnancy in a resource-constrained setting. We found the disparity in the care of surgically-treatable conditions is epitomized by the treatment and clinical course of ectopic pregnancy throughout the world.
Because of the potential to decrease morbidity and mortality for women who are critical to the success and vitality of nations, we chose to develop a module to teach the laparoscopic treatment of ectopic pregnancy. With ALL-SAFE, our goal is learners will become competent and confident in performing all aspects of patient care in the laparoscopic treatment of ectopic pregnancy.
Expansion of ALL-SAFE[edit | edit source]
We found the proficiency in our original module would confer a skillset that would be applicable towards performance of laparoscopy in the resource-constrained setting in general, inclusive of common diseases. Since the launch of our original module, we have successfully launched modules on laparoscopic management of appendicitis, penetrating thoracoabdominal trauma, and Meckel’s diverticulum. Our next module in-development focuses on trocar placement and establishing working space.
By creating a comprehensive simulation, each module supports the development of both cognitive and technical skills required for basic management of these conditions. We cover learning from the initial clinical encounter to post-operative recovery through an interactive clinical vignette. This is followed by the psychomotor skills training with the participant performing the procedure using our low cost, easy-to-build box trainer simulation. Each module highlights a different technical skill fundamental to laparoscopic surgery such as instrument handling, intracorporeal knot tying, and handling small bowel. In addition to self-assessment, learners will be able to share their results with a global community, thus receiving further feedback from peers and experts.
Today, the ALL-SAFE Consortium includes members from Pan-African Academy of Christian Surgeons, University of Michigan, Southern Illinois University, Soddo Christian Hospital, AIC Kijabe Hospital, and Mbingo Baptist Hospital. We hope to continue promote collaboration between countries of diverse resource levels to provide a low-cost, portable educational and assessment framework to improve the quality of surgical care.
Our Target Sites[edit | edit source]
This is a collaborative map of our pilot sites as well as places where our platform has been used so far.
Our target users/learners are trainees and residents in low- and middle-income countries. Our pilot sites are 3 PAACS residency program sites: Mbingo, Cameroon; Soddo, Ethiopia; and Kijabe, Kenya. The platform has been rolled out to all PAACS sites.
