This page provides a psychomotor assessment rubric for evaluating surgical scrubbing competency, developed for perioperative nursing training under ECSACONM (East, Central and Southern Africa College of Nursing). It is a companion assessment tool to the Surgical Scrubbing Skills Practice Instructions page, used by evaluators to rate student/practitioner performance on each step of the surgical scrub on a 5-point scale.

Purpose

To provide a standardized scoring framework for assessing clinical competency in surgical scrubbing, ranging from "Clinically Unacceptable / Not Performed" (1) to "Excellent" (5), with defined criteria at each anchor point.

Rating Scale

  • 1 – Clinically Unacceptable / Not Performed
  • 2 – (between 1 and 3, no separate descriptor)
  • 3 – Clinically Acceptable / Performed
  • 4 – (between 3 and 5, no separate descriptor)
  • 5 – Excellent

9 1st, 2nd and 3rd scrub: Scrub hands, forearms and elbows Observed Uses a non-systematic technique, scrubs from elbows toward the hands, misses required surfaces, lowers the hands below the elbows, or fails to complete the required scrub cycles or duration Applies the correct amount of scrub solution, maintains hands above elbows, scrubs systematically from the hands toward the forearms and elbows using circular motions, and completes the required scrub cycles according to local protocol Performs all scrub cycles confidently and consistently with excellent technique, complete surface coverage, correct directionality, appropriate antimicrobial contact time, and continuous contamination awareness throughout the procedure
