This page provides a psychomotor assessment rubric for managing orthopedic surgery, developed for perioperative nursing training under ECSACONM (East, Central and Southern Africa College of Nursing). It accompanies the corresponding psychomotor skills practice worksheet and is used to assess learner performance across each step.

Each step is assessed by one of two methods. Articulated steps are scored as either "Not done" (clinically unacceptable / not performed) or "Done" (excellent), with no intermediate levels. Observed steps are scored on a five-point scale, with performance anchors defined at level 1 (clinically unacceptable / not performed), level 3 (clinically acceptable / performed), and level 5 (excellent); levels 2 and 4 represent performance between the defined anchors.

Instrument layout is highly organized, supports efficient orthopedic workflow, anticipates procedural progression, and allows immediate access to all required items without searching.
4 Set up and test orthopedic drill Observed Drill is improperly assembled, wrong drill bit is selected, attachments are unsecured, functionality is not tested, or backup equipment is unavailable. Drill is assembled correctly, tested successfully, proper attachments are secured, and backup equipment is available. Drill system is assembled efficiently, all safety checks are completed proactively, rotation/function are verified, and backup systems are confirmed without prompting.
5 Prepare pins, rods, clamps, and external fixator components Observed Components are disorganized, incorrect sizes are selected, compatibility is not verified, or defects are missed. Pins, rods, clamps, and connectors are organized appropriately, inspected, and verified for compatibility and readiness. Components are organized systematically by type and size, defects are identified proactively, and all likely required options are prepared efficiently.
6 Load and verify suction and irrigation setup Observed Suction or irrigation systems are improperly connected, not tested, nonfunctional, or unavailable when needed. Suction and irrigation systems are connected, tested, functional, and positioned appropriately for use. Equipment is assembled efficiently, flow is verified, tubing is secured, and systems are positioned to support uninterrupted workflow.
7 Assist with draping and limb positioning Observed Limb is inadequately positioned, drapes are insecure or contaminated, exposure is inadequate, or sterility is compromised. Limb is positioned appropriately, drapes are secure, exposure is adequate, and sterility is maintained. Limb positioning optimizes both surgical access and imaging access, adjustments are anticipated appropriately, and sterility is maintained throughout.
8 Assist during drilling and pin insertion Observed Required instruments are unavailable when needed, workflow is disrupted, assistance is delayed, or organization is poor. Required instruments and supplies are available when needed, workflow is maintained, and assistance supports the procedure appropriately. Anticipates procedural needs accurately, provides instruments smoothly and efficiently, and supports uninterrupted drilling and pin insertion workflow.
9 Assist with external fixator frame assembly Observed Incorrect components are provided, assembly sequence is not supported, or workflow is significantly delayed. Correct components are provided in an appropriate sequence and assembly proceeds without unnecessary delays. Consistently anticipates assembly requirements, provides components proactively, and supports efficient and accurate frame assembly.
