Use this template to embed Google Forms.

Usage[edit source]

Open the Google Form you want to embed.
Copy the form ID from the URL. For example if the URL is https://docs.google.com/forms/d/e/1FAIpQLSdr0iPUCOzgFqbBRnHgpiQp1fS8JZDt43aBA-yH8aDoTyH2Qg/viewform then the form ID is 1FAIpQLSdr0iPUCOzgFqbBRnHgpiQp1fS8JZDt43aBA-yH8aDoTyH2Qg
Paste the following wikitext anywhere in Appropedia, replacing the example form ID for the real one:
