Usage

{{PD source|source=[[http://foo/ http://foo] bar]|statement=http://foo2/}}

After "source=" place the link to source (and description if desired).

After "statement=" place a raw link (just the url to public domain/copyright-free statement). If there is no such statement on the site, but it is clearly public domain (usually because it is the work of a US federal official) then use the following link:

http://www.appropedia.org/US_federal_government_websites_and_public_domain

E.g. the following code:
